To participate, simply fill out the form and email it, along with your high-resolution photo(s), to Seymourhelpinghands@Gmail.com. If you need a form, we’re here to assist! Request one through Messenger by sending us your email address, email us directly, or visit the SHH Community Center at 100 S Washington to grab a form in person.
